About In Home Rehabilitation
Everhome Healthcare was established over 18 years ago by a team of healthcare professionals with collectively over 100 years of healthcare delivery, administration and research experience.
Our goal is to help individuals make progress on their goals through construction of a care plan developed and managed by licensed physical therapists and enacted by trained certified staff.
What to expect
1. The Therapist reviews medical records to thoroughly understand course of treatment.
2. The Therapist collaborates with client, family and primary care provider regarding goals and expectations as part of the initial evaluation.
3. After evaluation, the Physical Therapist establishes the plan of care, reviews it with client, family and the care coordinator to align expectations and initiates a training session covering the plan of care.
4. The Care Coordinator records the training session in writing or by video.
5. Care Coordinator introduces care giver(s) to the client and trains them on the plan of care
6. The physical therapist does a weekly on-site check in with the care coordinator or caregiver in attendance.
7. Assessment of the client's progress and changes are reviewed weekly in a case conference
8. Plan of care is edited or amended as indicated by the weekly case conference

1. Preparation:

Review Hospital and Home Health medical records: Pre-acute event; Acute hospitalization; Subacute; Home Health

Collaborate with client, family, and primary care provider regarding goals and expectations

Physical Therapist establishes a plan of care
2. Care Coordinator training:

Physical Therapist evaluates the patient and develops a goal oriented plan of care.

Physical Therapist reviews the care plan with Care Coordinator and client/family to align expectations

Physical Therapist initiates the training session after the therapy evaluation.

Care Coordinator records intervention in the care plan in writing or video

Video or documents are made available to caregivers and family through YouTube videos or handouts
3. Caregiver training:

Care Coordinator sends YouTube video or handouts to Caregivers

Care Coordinator introduces every Caregiver to the family and trains in the plan of care, including reviewing video trainings or handouts
4. Weekly Plan of Care monitoring:

On-site check-ins by licensed clinicians with experienced Care Coordinator and/or caregiver

Weekly Case conference discussion

Clinician assessment of progress

Plan of care edits, as indicated by weekly assessments
